Two routes, two chemistries

Swallowed testosterone is destroyed by the liver before it reaches the blood. Chemists solved this two ways:

  • Injectables: attach an ester so the hormone releases slowly from an oil depot. The liver is bypassed at entry, and the molecule that circulates is the parent hormone.
  • Orals: add a methyl or ethyl group at carbon 17 so the liver cannot break the molecule down easily. It survives, and the liver processes a molecule it struggles with, every day.

Why physicians choose injectables

The 2018 Endocrine Society guideline on testosterone therapy is built around injectable and transdermal testosterone precisely because their liver and lipid profile is manageable with monitoring. Alkylated orals do not appear in replacement therapy for the same reason they appear in case reports of cholestasis.

If an oral is used at all: liver panel before and during, with the right markers. ALT and AST rise with heavy training and mislead. GGT and bilirubin point to the liver itself. What injectables do not fix
