In short

Libido and drive change within the first three weeks and plateau by six. Water and fullness show in the first week or two. Strength climbs from week two or three. Measurable muscle and fat changes appear at 12–16 weeks and keep accruing to six or twelve months. Red cell mass rises from three months and peaks at nine to twelve. Lipid changes appear by four weeks. Judging a regimen at week four is judging water and mood, not muscle.

Key facts
  • Sexual interest changes appear at about 3 weeks and plateau at 6 (Saad et al., systematic review, 2011).
  • Changes in lean mass and fat mass become measurable at 12–16 weeks and continue to 6–12 months.
  • Erythropoiesis increases from about 3 months and peaks at 9–12 months, which is why hematocrit keeps rising with continuous use.
  • Lipid changes are detectable at about 4 weeks, with maximum effects after 6–12 months.
  • In the 1996 controlled trial, 10 weeks of testosterone plus training added about 6 kg of fat-free mass, so the bulk of visible change is a months-long process.

The timeline

Saad and colleagues pulled every treatment study with repeated measurements and mapped when each effect first appears and when it stops increasing. Supraphysiological use shifts the size of the effects, not the order or the clock.

Effect First measurable Plateau or maximum
Sexual interest, drive ~3 weeks ~6 weeks
Water retention, fullness 1–2 weeks 4–6 weeks
Erections, mood, energy 3–6 weeks 3–6 months
Strength 2–4 weeks keeps rising with training
Lean mass 12–16 weeks 6–12 months and beyond
Fat mass 12–16 weeks 6–12 months
Estradiol-driven effects 2–4 weeks 4–8 weeks
Lipids (HDL down) ~4 weeks 6–12 months
Hemoglobin, hematocrit ~3 months 9–12 months
Bone density 6 months 3 years

What that means in practice

Weeks 1–3. Water, glycogen, pump, drive. The scale jumps. None of it is muscle yet. Blood pressure can already move; start measuring now, not later.

Weeks 3–6. Levels reach steady state with a medium ester. Libido and mood plateau. Strength starts climbing as neural drive and recovery improve. Estradiol effects, if any, show here: nipple sensitivity, puffiness. First lipid changes are measurable.

Weeks 6–12. Real tissue accrues. The 1996 trial measured about 6 kg of fat-free mass at week 10 with training. Fat starts dropping if food and training allow. The early euphoria settles; do not mistake that for the compound “stopping working”.

Months 3–6. Lean mass and fat changes become obvious and measurable. Hematocrit begins its climb; this is the first blood draw where it matters. Strength continues if training progresses.

Months 6–12. Effects on body composition approach their maximum for a given exposure. Hematocrit peaks at 9–12 months, which is the core reason continuous use accumulates risk while defined periods do not.

Reading your own progress

  • Judge body composition at 12 weeks, not 4. Use waist, photos and gym numbers with the scale.
  • Test the trough at 4–6 weeks, then hematocrit and lipids every 2–3 months. Their clocks are slower than muscle’s.
  • The plateau in how you feel arrives long before the plateau in what you build.

After stopping

Effects fade in reverse: water and drive within weeks, strength slowly if you keep lifting, muscle over months toward your natural set point, lipids and hematocrit back over weeks to a few months. How to keep the most of it.

Frequently asked questions

How fast do you see results on testosterone?

Water, fullness and drive in the first one to three weeks; strength from week two or three; visible, measurable muscle from about three months, still increasing at six to twelve. Early size is mostly glycogen and water.

When should I judge whether it is working?

Not before 12 weeks for body composition. Blood levels stabilise at four to six weeks with a medium ester; muscle follows months later. Judging at week four measures water and mood.

Why do I feel great at week two and normal at week eight?

Mood and libido effects plateau early, around six weeks, while the body adapts to the new level. Muscle keeps building after the feeling flattens.

When do side effects show?

Water and blood pressure within weeks; estradiol effects within weeks; lipid changes by four weeks; hematocrit rises from about three months and keeps climbing to nine or twelve. That is why the risk of continuous use is a slow burn.

How long after stopping do effects fade?

Water and drive within weeks as levels fall; strength holds longer if you keep training; muscle declines over months toward what your own hormones support; hematocrit and lipids normalise over weeks to a few months.
